Osteoarthritis & Joint Health
Every kilogram of body weight removed reduces knee joint load by four kilograms.

The letters on this page
Nine interventions on this page. Five carry a letter; four are listed without one, with the reason.
How it is measured
What has been graded
4:1 mechanical load reduction per kg lost. The single most impactful intervention for knee and hip OA. Evidence is consistent across multiple large RCTs Randomized controlled trial: people are assigned to the treatment or a comparison by chance, so the groups differ only in what they received. It is the design that can show cause and effect..
Not graded for this use. Not separately graded. EBL grades interventions at a studied dose and protocol, and this entry has no authored record behind it yet.
Pain and physical function improvement across multiple meta-analyses A study that pools the numbers from several trials into one combined estimate.. Low-impact aquatic exercise is particularly well-tolerated in severe OA.
Not graded for this use. Combination. EBL grades an intervention at the dose and protocol a trial studied. No trial has studied this combination as a combination, so it carries no letter; the components are graded separately.
Synovial inflammation reduction. Pain score improvement in OA RCTs. Mechanism: reduced prostaglandin E2 and leukotriene B4 synthesis.
Not graded for this use. Omega-3's graded uses are cardiovascular event endpoints.
GAIT trial and GUIDE trial evidence. Important: use glucosamine sulfate (Rotta pharmaceutical form), not hydrochloride. HCl form has not shown consistent benefit.
Cartilage thickness RCTs. Undenatured type II collagen also has RCT evidence via oral tolerance mechanism. Both forms are studied.
Not graded for this use. Collagen peptides are graded B on activity-related joint pain in athletes and active adults; the trials did not enroll people with diagnosed osteoarthritis, so this row carries no letter.
Noninferior to ibuprofen 1,200mg/day on WOMAC pain and function in 367 patients (PMID 24672232); an 11-trial meta-analysis of low overall study quality found curcuminoids beat placebo for pain and function (PMID 29622343). Neither head-to-head trial had a placebo arm. The trials used unenhanced curcuminoid extract at 1,500-2,000mg/day, not an enhanced-bioavailability form at a lower dose.
5-LOX inhibition reduces leukotriene-driven joint inflammation. Multiple RCTs show significant pain and function improvement. AKBA is the active constituent.
Emerging RCT evidence for joint lubrication and synovial fluid viscosity. Distinct from injected HA. Early human data is promising but limited in size.
Cautions
- •Long-term NSAID use (ibuprofen, naproxen, celecoxib) may increase cardiovascular risk. The minimum effective dose and duration are a decision for your physician.
- •Repeated corticosteroid injections have been associated with greater cartilage loss. How often is a decision for your physician.
- •Glucosamine has been reported to raise INR in people taking warfarin, and its effect on insulin sensitivity is small and inconsistent.
- •Omega-3 fatty acids can modestly affect platelet function; large cardiovascular trials generally have not shown a major increase in serious bleeding. High-dose regimens can increase atrial fibrillation risk.
This page is educational only. OA diagnosis and severity staging require clinical and imaging evaluation. Before starting glucosamine, discuss with your physician if you are diabetic or on anticoagulants. The Drug-Nutrient Depletion tool checks current medications before anything new is added.
