In plain terms

In plain terms: Infrared Sauna shows early signals in human research but the evidence is limited -- small trials, surrogate endpoints, or not yet independently replicated. It is an emerging area, not yet a proven one.

The evidence as the record states it

Tei et al. (Circulation Journal, 2016; PMID 27001189, Waon therapy), Masuda et al. (Japanese Heart Journal, 2004; PMID 15090706), Hussain et al. (Complementary Therapies in Medicine, 2022; PMID 34954348) and Schenaarts et al. (2024; PMID 39209309): randomized trials of sauna/Waon therapy. Grade C: one heart failure trial whose primary endpoint was null, and marker trials that are small and mixed.

Evidence by specific use

Chronic heart failure (adjunct)
Grade C
Tei et al. (Circ J, 2016; PMID 27001189), WAON-CHF: 149 hospitalized patients with advanced heart failure, 10 days of Waon therapy or control. The primary endpoint, B-type natriuretic peptide, did not differ significantly; NYHA class, six-minute walk distance and cardiothoracic ratio improved. One trial, its primary endpoint null, not repeated by an independent group: C.

Vascular / oxidative-stress markers
Grade C
Masuda et al. (Jpn Heart J, 2004; PMID 15090706): 28 patients with coronary risk factors, 2 weeks; a urinary oxidative-stress marker and systolic pressure fell. Hussain et al. (Complement Ther Med, 2022; PMID 34954348): a single-session crossover in 10 women, no change in blood pressure or arterial stiffness. An oxidative-stress marker is not an established stand-in for a clinical outcome, and the evidence is one small positive trial against a null one: C.

Why Grade C and not Grade B

Grade C reflects early or limited human evidence -- small trials, surrogate endpoints, or preliminary findings not yet independently replicated. The evidence points in a promising direction but does not yet meet the standard for Grade B. A single well-powered, independently replicated trial with a meaningful endpoint would move this to Grade B or higher.

What this grade covers and what it does not

Grade C. The heart failure use rests on one multicenter trial, WAON-CHF (Tei et al., Circ J, 2016; PMID 27001189), whose primary endpoint, B-type natriuretic peptide, did not differ; functional class and walk distance improved as secondary outcomes. The vascular use rests on a 28-person, 2-week trial on a urinary oxidative-stress marker (Masuda et al., 2004; PMID 15090706) against a null single-session crossover (Hussain et al., 2022; PMID 34954348). Neither meets the B definition. WHAT THIS GRADE DOES NOT COVER: mortality, where the Finnish sauna data are observational and carry no letter.

What would move this grade

A well-powered randomized trial with an objective, clinically meaningful primary endpoint and at least one independent replication would support Grade B or higher.

How to track whether this is working Resting heart rate and blood pressure at baseline and every 4 weeks. Session frequency, temperature, and duration to confirm the protocol matches studied parameters (4+ sessions/week, 174-212F, 20 minutes). If using for recovery, track DOMS scores and performance recovery time across training blocks.
